INSTRUCTIONS -

1) Parent can read the question to the child .

2) Student will either type the answer himself , or may need parent's help to do so.

3) Read carefully and answer all the questions .

4) The student can submit only one response . If he uses another phone/ laptop/emailsecond time , it will be nullify .
5)After 24 hours , the response window will be freezed .
